LOUDOUN COUNTY, Va. (WUSA) -- The Loudoun County sheriff's office has identified the utility contractor who was killed Tuesday night in multi-vehicle accident at the intersection of Rt. 7 and Blue Ridge Mountain Rd/Rte. 601.

Authorities say 57-year-old Jacqueline R. Green of Milton, Florida was driving a bucket truck that was pulling a service trailer at approximately 7:20 p.m. Tuesday when it appears there were issues with the brakes. Witnesses told authorities that they saw the truck go from the northbound lane into the southbound lane of Blue Ridge Mountain run, passing a truck at the front of a convoy of utility vehicles.

The truck then went down the hill towards the intersection with Route 7, coming into the eastbound lanes of Route 7. There, the truck struck "a 2008 Freightliner trucktractor-semitrailer," say sheriff's office authorities. That vehicle then stuck the side of a Toyota Tacoma.

Green died at the scene. The front seat passenger in her truck was taken to Winchester Medical Center. Authorities say the driver of the Tacoma and passenger were treated and released at a local hospital. The driver of the semi trailer was also taken to the hospital.

Green part of a convoy of three Gulf Power trucks from Florida that were in the area helping Dominion Virginia Power crews repair downed lines and restore power.

The cause of the accident is under investigation.
